This Notice applies when you use L'vae, a Praventis Group LLC Company ("L'vae," "we," "our," or "us"), to receive telehealth services that involve your Protected Health Information ("PHI").

- L'vae
- our licensed telehealth provider
- our licensed pharmacy partner
How We May Use and Disclose Your PHI
HIPAA permits us to use or disclose your PHI for certain purposes without your additional written permission.
Treatment
We may use/disclose PHI to provide, coordinate, or manage your care.
Examples:
- reviewing your consultation
- prescribing medication
- dispensing medication through our licensed pharmacy partner
Payment
We may use/disclose PHI to process payments for services you receive.
Examples:
- verifying orders
- processing subscription renewals
- billing your chosen payment method
(L'vae is cash-pay only and does not bill insurance.)
Healthcare Operations
We may use/disclose PHI for activities necessary to operate our service.
Examples:
- quality reviews
- clinician oversight
- regulatory audits
- customer support
Other Uses and Disclosures
We may also use or disclose PHI in the following situations:

If federal or state law requires it.

De-Identified Information
We may remove all identifiers from your PHI and use/share the de-identified data for lawful purposes.

We may work with third-party vendors who help us deliver services (secure hosting, support tools, etc.)
All vendors are bound by HIPAA-compliant agreements.
Your HIPAA Rights
You have the following rights regarding your PHI.
Right to Access
You may request a copy of your medical records.
Right to Request Amendments
You may request corrections to inaccurate information.
Right to an Accounting of Disclosures
You may request a list of certain disclosures we have made.
Right to Request Restrictions
You may request limits on how we use or disclose PHI (we may not always be able to agree).
Right to Confidential Communications
We will communicate with you via your preferred method when possible (email, secure portal, etc.).
Right to a Paper Copy of This Notice
You may request a printed version at any time.
Right to File a Complaint
You may file a complaint with L'vae or directly with the U.S. Department of Health & Human Services (HHS).
We will not retaliate against you for filing a complaint.

We are required by law to:
- maintain the privacy of your PHI
- provide this Notice
- follow the terms of this Notice
- notify you if a breach occurs that affects your PHI
