These Terms & Conditions ("Terms") govern your access to and use of the websites, mobile applications, digital platforms, and related services operated by Praventis Group LLC, doing business as L'vae ("L'vae," "we," "our," or "us").
By accessing or using the Services, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree, do not use the Services.

About L'vae
L'vae provides a digital platform that enables users to connect with:
- An independent, third-party medical group ("Affiliated Provider Group") whose licensed healthcare professionals ("Providers") deliver telehealth services; and
- A licensed pharmacy partner that dispenses medications prescribed by the Providers.
L'vae itself does not provide medical care, does not employ doctors, does not practise medicine, and does not dispense medications. All medical decision-making is performed solely by independent Providers unaffiliated with L'vae.
Eligibility
To use the Services or receive telehealth care through the platform, you must:
- Be at least 18 years old
- Reside in a state where the Services are available
- Provide accurate and complete information
- Have the legal capacity to enter binding agreements
You must not use the Services on behalf of another person.
No Emergency Care
The Services must not be used for medical emergencies. If you believe you are experiencing an emergency, call 911 immediately.
Telehealth Acknowledgement
By using the platform, you agree that:
- Telehealth involves the remote delivery of healthcare services using audio, video, asynchronous messaging, and/or digital questionnaires.
- Telehealth has risks and benefits, including limitations related to physical examination.
- Providers may determine that telehealth is not appropriate for your condition.
- No specific outcome, resolution, or improvement is guaranteed.
You will be required to review and sign a dedicated Telehealth Consent before using clinical services.
Your Responsibilities
You agree to:
- Provide accurate, truthful, and complete information
- Update your medical history as needed
- Use medications only as prescribed
- Not share medications with any third party
- Notify your primary healthcare provider about treatments you receive
- Use the Services only for lawful and personal purposes
- Keep your login credentials confidential
Providing false or misleading information may result in harm and may violate state or federal law.
Platform Role & Relationship with Providers
You acknowledge and agree that:
- L'vae is not a healthcare provider
- Providers are independent professionals solely responsible for the medical care they deliver
- L'vae does not interfere with or control Provider clinical judgment
- Communications between you and Providers may occur through secure messaging, asynchronous questionnaires, audio, or video
- Providers may decline care if medically inappropriate
Prescriptions and Pharmacy Services
If a Provider determines that treatment is appropriate:
- A prescription may be issued in accordance with applicable state and federal laws
- Prescriptions are transmitted to a licensed pharmacy partner for dispensing
- L'vae does not guarantee that a prescription will be issued
- You may request your prescription be sent to a pharmacy of your choice if legally permitted
No Controlled Substances
Providers do not prescribe controlled substances through the platform, including but not limited to:
- Opioids
- Benzodiazepines
- Schedule II-V medications
Payment, Billing & Subscriptions (Cash-Pay Only)
Cash-Pay Model
All services, consultations, and prescriptions provided through the platform are self-pay only. We do not accept:
- Insurance
- Medicare
- Medicaid
- Government reimbursement programs
Subscription Plans
If you enroll in a subscription:
- You authorise recurring charges to your chosen payment method
- You may update, pause, or cancel your subscription anytime through your account
- Cancellation must occur before the next billing cycle to avoid further charges
Payment Processing
Payments are handled by secure third-party processors. L'vae does not store full payment card information.
Refund Policy
Prescription Medications
Once a prescription is authorised and dispensed by the licensed pharmacy partner:
Prescription products cannot be returned or refunded, except in cases of:
- Pharmacy error
- Damage
- Incorrect product shipped
Non-Prescription Products
Unopened, unused, non-prescription items may be returned within 14 days of delivery.
Clinical Services
Once a Provider has reviewed your information, the consultation is considered delivered and is non-refundable.
Use of the Website and Platform
You agree not to:
- Engage in unauthorised access or data scraping
- Introduce malware or harmful code
- Copy, distribute, or adapt platform content
- Interfere with the operation or performance of the Services
Violation may result in account suspension or termination.
Intellectual Property
All materials on the platform, including text, design, graphics, logos, and content are owned by Praventis Group LLC and protected by US intellectual property laws.
You may not reproduce or use content for commercial purposes without prior written consent.
Privacy & Data Protection
Your use of the Services is governed by our Privacy Policy, which explains:
- What information we collect
- How it is used
- How it is shared
- Your rights
Although L'vae is not a "covered entity" under HIPAA, we maintain strict administrative, technical, and physical safeguards consistent with industry best practices.
External Links
L'vae may link to third-party sites. We do not endorse and are not responsible for their content or privacy practices.
Limitation of Liability
To the fullest extent permitted by law:
- L'vae is not liable for indirect, incidental, special, punitive, or consequential damages
- L'vae is not responsible for delays, interruptions, or failures caused by external systems or third parties
- L'vae makes no guarantees regarding clinical outcomes
Nothing in these Terms limits liability for:
- Death or personal injury caused by negligence
- Fraud or fraudulent misrepresentation
- Other liabilities that cannot legally be limited
Indemnification
You agree to indemnify and hold harmless Praventis Group LLC, its officers, employees, and agents from claims, damages, or losses arising from:
- Your misuse of the Services
- Your violation of these Terms
- Your breach of state or federal law
Modifications
We may update these Terms from time to time.
Changes will be posted with a new "Last Updated" date.
Your continued use of the Services constitutes acceptance of the revised Terms.
Governing Law & Dispute Resolution
These Terms are governed by the laws of the State of Delaware, excluding conflict-of-law principles.
Any disputes must be resolved through:
- Informal resolution first, then
- Binding arbitration, unless prohibited by law
You waive the right to participate in class actions.
